The Nutrition Educator will provide telephonic nutrition education to members with chronic conditions as they learn to select, prepare, and eat foods that are appropriate for their condition(s) as they participate in a chronic care meals program. Chronic conditions can include, but are not limited to diabetes, heart disease and kidney disease. Utilizing a member-centric approach, the Nutrition Educator will collaborate with the member to explore the nutrition-related issues and topics that are most important to the member. The Nutrition Educator will also help connect members to other nutrition and condition resources available from the health plan or system or in their community.This is a PRN/temp position with the potential for a more permanent role as determined by the volume/business need after 90 days. Number of hours per week will vary but expect to work ~40 hours per week. Position Responsibilities may include, but not limited toProvide telephonic nutrition education sessions per member. Number of sessions offered varies, based on what program the member is enrolled in Primary member population is in California, so working hours for this position would be ~9am-6pm Pacific TimeDocument goals and care plan from education sessions in platformSchedule follow-up calls with each member/patientIdentify community resources for members as neededMaintain and update professional knowledge by participating in educational opportunities; reading professional publications; maintaining personal networks; participating in professional organizationsRequired Skills and ExperienceBachelor of Science in Nutritional Science or Dietetics Certification as a Registered Dietitian by the Commission on Dietetic RegistrationMinimum of 2 years' experience providing nutrition education or counselingExcellent oral and written communication skillsStrong organizational and interpersonal skillsThis role routinely uses standard office equipment such as laptops computers and smartphonesWork independently and has a strong sense of responsibility carrying out the required job duties with a minimum amount of daily supervisionAbility to make clinical decisions as they pertain to the appropriateness of nutrition education for each member/patient Preferred Skills and ExperiencePrevious experience providing telephonic nutritional education or counseling Bilingual/Spanish speaking Training in health coaching, motivational interviewing, and intuitive/mindful eating framework Physical RequirementsRepetitive motions that include the wrists, hands and/or fingersSedentary work that primarily involves sitting, remaining in a stationary position for prolonged periodsVisual perception to perform job including peripheral vision, depth perception, and the ability to adjust focusCompany OverviewMom's Meals is a home-delivered meal service providing fully prepared, refrigerated meal solutions direct to homes nationwide for over 25 years.
